Prospective patients must have a referral from a doctor. Pertinent medical records must be forwarded for review. Dr. Charles Buzzanell, owner and medical director of Blue Ridge Pain Management, reviews every referral. Appointments are made following a careful review. Please print the referral form (click to download) and have your doctor complete and fax it with your medical records to begin the review process prior to setting up an appointment with our office.
